Monthly Memberships

Automatic monthly billing will be setup on the the date in which the membership was purchased. Billing continuously occurs every month for a minimum of 12 months on such date until cancellation in writing is received by Driven Elite Fitness & Health (DEFH). Cancellation can be made any time before the billing cycle but once billing occurs and the client's account is charged, there is no refund for any reason. If serious injury or illness occurs, and proof of a doctor's note is provided, DEFH, may allow for a credit to be given to that client's account or any immediate family member. Credits are determined by the amount of the membership that is remaining. DEFH also allows for the client's account to be placed on hold for up to two months, if, and only if, serious injury or illness occurs. All other excuses/reasons are invalid. After two months, the client’s monthly payments will begin drafting again. If, for any reason, the client’s monthly payment is declined, a $25 service fee and the membership amount will be payable prior to the client attending any classes or training sessions.. Classes or training sessions paid for, but not used do not relieve the client of their obligations under the terms of agreement. All sessions must be completed within the purchase period time frame (example: A VIP monthly membership allows for a max of 15 sessions/month at three sessions/week that must be completed within each of the respective months); sessions do not roll over and will not be transferred to another membership. Memberships will not be extended due for unused sessions unless there is proof of the aforementioned circumstances.
